What is this program about?
Smart Living is an incentive program provided by Smith Moore Leatherwood that focuses on self awareness and personal health management for your well-being, not just physical health. You can find all the information you need to participation this site, including steps to assess your current state of well-being.
Why should I take part in this if I already lead a healthy lifestyle?
Good for you! Participation in the Smith Moore Leatherwood is entirely voluntary. Since you're already choosing healthy habits, you likely do not have health risks that are controllable. The screening can reveal important health risks—some of which are influenced by other factors and are not always apparent. We are making this program available to increase self awareness of your current health status and what you can do to reduce your risks.
Who is ADURO and Limeade?
Smith Moore Leatherwood has partnered with ADURO as the third party provider for our wellness services—primarily to protect employees' privacy. The online content on our custom website is powered by Limeade, a partner with ADURO. All personal health information is securely stored on their server; Casey Family Programs does not have access to your personal information—only aggregate data.
Will my health status make it hard for me to get benefits?
Participation in the health screening is voluntary and has no bearing on your access to health benefits. However, in order to receive Wellness points, you must complete a biometric screening. Wellness points are solely based on your participation, not your results. Any personal information is completely confidential—Smith Moore Leatherwood will only know aggregate information about the entire workforce.
What if I'm not able to do the health screening?
We are making the on-site health screenings available for employees' convenience to "know their numbers" as part of the assessment. Here are alternative options: order a home test kit, or contact your provider if you've had a wellness visit in the past year to get your key lipid measurements and blood pressure. You can also check your blood pressure at many retail pharmacy locations.
Who is eligible to take part in the onsite health screenings?
All Smith Moore Leatherwood employees are eligible to receive free screenings.
Why are these screenings important?
As part of Smith Moore Leatherwood , these on-site health screenings are about knowing your numbers and taking steps to learn more about, and improve, your health.
How private are these screenings and results?
Screenings will be performed in a completely private setting. Any Personal Health Information you supply is for your eyes only. Smith moore Leatherwood respects your privacy and will not receive your individual results under any circumstance. It is protected by federal and state privacy laws including the Health Insurance Portability and Accountability Act (HIPAA). So relax and enjoy this opportunity to know your numbers and earn 250 Wellness points.
How will the data be used?
Only you see your individual results: it is intended to increase your awareness of personal health risks so you can better manage your health. No personal data is shared with Casey Family Programs. The aggregate data will be available to Smith Moore Leatherwood so that we can identify specific health management programs that may be appropriate.
Will I incur any out of pocket costs for the screening?
The entire cost of this program, including online and telephonic coaching resources is covered by Smith Moore Leatherwood.
What do I need to do to prepare for the screenings?
The cholesterol/glucose test is a simple finger-prick test that will provide you with immediate results. We do ask that you refrain from consuming any food or beverages (other than water) 2–4 hours prior to your test. This will help ensure a more accurate reading. |
