What is This?
Live Well Work Well is a wellness incentive program that focuses on self awareness and personal health management for your total well-being, not just physical health. You can find all the information you need to participate on this site, including steps to assess your current state of well-being.
Why am I doing this?
As part of the new Live Well Work Well Wellness program, these on-site health screenings are about knowing your numbers and taking steps to learn more about and improve your health.
Is it Private?
Any Personal Health Information you supply is for your eyes only. We respect your privacy and will not receive your individual results under any circumstance. It is protected by federal and state privacy laws including the Health Insurance Portability and Accountability Act (HIPAA). So relax and enjoy this opportunity to learn your numbers.
What's next?
With your health numbers you will be able to know if you are at risk for developing either cardiovascular disease or a diabetic related condition. If you fall outside the normal range, please make an appointment with your physician to discover what the next steps are. If you are within normal ranges then you should continue to pursue the personal lifestyle choices that will keep you healthy and well. Live Well Work Well Wellness is here to support your physician's instructions for a healthy lifestyle. |
